Please do not forget your health insurance card. If you do not present your health insurance card, you will be responsible for the full amount of your medical expenses.

・Medication notebook, etc. (if you have one)
If you are receiving a prescription for medication from another medical institution, please bring your “Medication notebook” or
document that shows the contents of your medications, such as a “form listing your medications.”

Complete the necessary items on the medical examination application form and submit it with your “My Number Card or health insurance card” along with your
“referral letter” and “medication notebook” to the reception desk.
Once reception is completed, you will be given a “consultation ticket” and a “reception slip.”
Please wait in front of the department,
and when it is your turn, you will be called to enter the examination room.
Depending on the nature of the medical treatment, patients may be required to undergo tests in the laboratory or radiology department.
Please submit the “Outpatient check-in slip” to the cashier after completion of the consultation, examination, or test.
You will receive an “account number.” Please wait in front of the cashier.
When your account number is displayed on the “Account Number Display Board,” please make your payment using the automatic checkout machine.
Your “Prescription Number” is present on the right-hand side of your receipt.
Please wait in front of the pharmacy.
Please come to the pharmacy counter when your prescription number appears on the “Prescription Waiting Display Board.”
