Admission procedures

On the day of admission, the patient and two guarantors are required to sign and seal the “Admission Pledge.”
Please ensure to have your personal seal (inkan) ready for admission.In principle, the two guarantors should be from different households.
Please ensure to notify us of any changes to the information on the “Admission Pledge” form.

If there are any changes to the 'Hospitalization Agreement,' please be sure to notify us.

What to bring with you when you are admitted to hospital

Please minimize the amount of personal belongings that you bring with you (an amount that can fit into the chest of drawers in the hospital room).
We are not responsible for any loss, damage, or theft of personal belongings.

*We keep a record of all belongings received for safekeeping at the time of admission. Please notify the staff if you add new clothes or towels or discard or otherwise change them after hospitalization.

*Please write your full name on all your belongings with a permanent marker.

・Various medical care recipient certificates

・Certificate of Eligibility for Maximum Amount (for those under 70 years old)

・Certificate of Eligibility for Maximum Amount and Standard Cost Reduction (for those aged 70 and above from non-taxable households)

・Discharge certificate (for individuals who were hospitalized in another medical institution within 3 months)

・Documents related to new COVID-19 vaccination (for those who wish to be vaccinated and those who have been vaccinated)

・Current medications (please bring all medications that you are taking regardless of whether they are prescribed by our hospital or another)

・Personal seal (which will be returned immediately after stamping)

Check and submit to the cashier or at the Community Liaison Office

・Nursing-care insurance card

Submitted to the ward

・Admission Pledge (provided at the time of completing the hospitalization contract)

・Other items requested by the nurse for preparation (explanations will be provided after admission for those who need them)
 Examples: Pillows for position changes, items for physical restraints, surgical preparation items, etc.

Please bring the following items with you:

・Toiletries (toothbrush, toothpaste, denture case, electric shaver, etc.)

・Underwear and socks (4–7 pairs each)

・Tissue papers

・Plastic cups (one for meals and one for oral care), drinking straws, meal aprons, etc.
(for those who need them)

・Footwear (slippers: non-slip, sport shoes: for rehabilitation)

・Portable toilets (only for those who need them).
 Can be rented for short periods of time.

Items prohibited from being brought into the hospital

・Sharp objects, such as scissors, cutters and knives, and other dangerous items, such as weapons, firearms, and explosives

・Alcoholic beverages, cigarettes, and lighters

・Fresh flowers and plants (as they can cause infection and allergies)

Large amounts of cash and valuables (including ornaments)

The hospital is not responsible for any theft or loss.
Please manage and store valuables at your own responsibility (If you wish to deposit cash with us, we will manage it through a passbook, so please bring your Tokushima Taisho Bank passbook and registered seal).

You cannot consult or receive medication from other medical institutions while you are hospitalized.

・As a rule,it is not possible to use your health insurance card to consult other medical institutions (other than dental institutions) during hospitalization.
 The same is true for family members receiving medications from the family doctor or other health care provider on behalf of the patient.

・For medical appointments or consultations at facilities other than our hospital or if a specific department is unavailable, please consult with your primary doctor or nurse.

Car Parking

Car parking at the hospital is free of charge. As a rule, we ask inpatients to refrain from parking.
Please let us know if you have to park for an extended period due to unavoidable circumstances.

Important: Please note.

Please strictly adhere to the following rules and focus on a comfortable recovery.
・If you would like an explanation of your condition during your hospital stay, please speak to a nurse. We will coordinate an appointment with your doctor and contact you again. ・During hospitalization, please follow the instructions of the doctors and nurses. If you do not follow instructions and cause a nuisance to other patients or interfere with medical treatment, you will be discharged immediately. ・Depending on the patient's condition, they may be transferred to a different room within the general ward, or to a convalescent ward or a nursing care medical facility for treatment, nursing, or caregiving purposes. ・No smoking area is provided, as smoking is not permitted on the hospital grounds. ・Permission from the attending physician is required for all outings and overnight stays. If you wish to do so, please ask a nurse. Overnight stays are limited to a maximum of three consecutive days and six total days per month. ・Lights out is at 9 pm. After lights out, please turn off the TV/radio and sleep quietly. ・The following actions are strictly prohibited in the patient rooms:
  ✖ Frequent entry or exit to other rooms and noisy behavior
  ✖ Alcohol consumption and smoking (resulting in immediate discharge)
  ✖ Hanging laundry in the patient room
  ✖ Posting notices or using nails on walls
  ✖ Bringing unnecessary items into the room
・If you wish to bring in electrical appliances, please consult the nurse.

During your Hospital Stay

Meals

Breakfast, 7:30;   Lunch, 12:30;   Dinner, 18:00

As a rule, meals are served in the dining hall. For individuals who do not use the dining hall, staff will deliver and serve food to their rooms.

Bathing and changing bed sheets

Bathing days vary from patient to patient. An explanation will be given by a nurse after admission.
As a rule, those who need assistance with bathing are bathed twice a week.
Until the attending physician's approval is obtained, the patient will receive a bed bath.Sheets are changed once a week.

Visitations

Because of the increase in the number of new COVID-19 infections nationwide, we have decided to cancel visits on the following dates.
Tele-visitations (by appointment only) will be available during this period.

■ From Tuesday, July 30, 2024 until the end of the epidemic
For tele-visitation appointments and inquiries, please contact ☎ 088-668-1082.

■ Visiting days: Weekdays only
■ Time: 13:00–15:00 (15 minutes)
■ Visitors: Family and cohabitants of junior high school-age and older
■ Number of visitors: Up to 2 persons
* Visits may be restricted because of the patient's physical condition or other factors.

* Physical condition check (please refrain from visiting if you have a fever, cough, runny nose, sore throat, or any other symptoms of illness)
・If you wish to have a tele-visitation (by appointment only), please contact the Regional Liaison Office (direct phone: 088-668-1082).
・For emergency visits after 17:00, please enter through the emergency entrance.
・Bringing snacks or other gifts may not be allowed due to the patient's condition and infection control measures. Please consult with the nurse.

Fees

Hospitalization charges are billed once a month and upon discharge.

・Calculations are made at the time of discharge and at the end of the month. An invoice will be delivered to the patient's room on the 10th of each month. Please bring your invoice and pay at the cashier on the first floor
 (Payment due: 20th of every month)

・Invoices will be mailed when visiting is restricted because of infection control measures.

At discharge

On the day of discharge, the invoice will be brought to your hospital room. If you are discharged from the hospital on a holiday or after hours, we may not be able to provide you with an invoice on the day of discharge.
* Please keep the receipt in a safe place, as it may be required for your final tax return, application for the advanced medical care allowance, etc.

※If there are any changes to your health insurance card or other documents during hospitalization, please notify the cashier on the first floor without delay. ※If you have any questions about the contents of your invoice or wish to discuss payment, please feel free to contact the cashier on the first floor.

Available only at Tokushima Taisho Bank. Please bring your passbook and personal seal of Tokushima Taisho Bank (Withdrawals are made on the 15th of every month).

What is the Maximum Amount Application Certificate?
It is a certificate that, when presented to a medical institution, proves that the payment at the counter will be limited to the self-payment maximum amount.
With the patient's consent, the maximum amount application category can be checked online at our hospital's reception.
It is not necessary to apply for a maximum amount eligibility certificate at each local government or workplace office (Please present your My Number Card or health insurance card).

Protection of Personal Information

In principle, the hospital does not respond to inquiries by telephone—such as regarding whether or not a patient is hospitalized or where a patient is discharged to.
Please let us know if you do not wish to have your visits restricted or your name posted at the entrance of your room.

We are committed to medical safety in the hospital, and we ask for your understanding and cooperation to ensure a safe stay in the hospital.

Efforts to Prevent Accidents Caused by Misidentification of Patients

・Wristbands during
hospitalization Patients are required to wear a “wristband” with a bar code on their wrists or other parts of the body, which will be used to identify patients before and during tests, injections, blood transfusions, surgeries, and other procedures.

・We will confirm your name.
Regardless of whether you are an inpatient or outpatient, we will ask you (or your family members) to state your full name at all times, including consultations, tests, injections, blood transfusions, and surgery. We will confirm this repeatedly and appreciate your cooperation.

Prevention of falls

Our hospital makes every effort to provide an environment that fully considers the risk of falls. However, changes in the environment due to hospitalization or declining physical strength and motor functions due to illness or injury may result in unexpected falls or a fall from the bed.

Behavioral restrictions including physical restraints.

Stays in an unfamiliar hospital environment can cause temporary delirium, worsening dementia, and unexpected behaviors, such as patients pulling out therapeutically important tubes on their own. Our hospital's basic policy is not to use physical restraints; however, we may implement behavioral restrictions, such as use of restraints, if they are necessary to carry out the treatment or to protect the patient from threatening their lives.
